Perform and document treatments according to physician orders, facility policies, and standards of care. Provide treatment and management of acute and chronic wounds, ostomies, stomas, pressure injuries, skin tears, and skin breakdown related to urinary or fecal incontinence. Monitor residents for changes in skin integrity and identify risks for skin breakdown and other complications. Communicate resident needs, changes in condition, and treatment outcomes with nursing staff and other members of the interdisciplinary team. Coordinate treatments with residents' daily schedules and plans of care. Ensure required documentation is completed accurately and timely. Participate in staff meetings, in-services, and education related to wound prevention and treatment. Work collaboratively with the interdisciplinary team to promote continuity and quality of resident care.
