RN MDS Coordinator — Experience Required Marshall Manor Nursing & Rehabilitation Center, an award-winning skilled nursing facility in Guntersville, Alabama, is seeking an experienced RN MDS Coordinator to join our care team. The MDS Coordinator oversees the resident assessment and care planning process, working closely with the interdisciplinary team to ensure accurate documentation, timely assessments, and individualized resident care.

Responsibilities:

Coordinate accurate and timely completion and submission of MDS assessments and Care Area Assessments (CAAs). Manage assessment schedules and Assessment Reference Dates (ARDs) according to applicable requirements. Coordinate interdisciplinary care plan meetings and ensure care plans reflect residents' current needs. Review clinical documentation to support accurate MDS coding. Monitor submission validation reports, resolve errors, and verify assessment acceptance before billing. Collaborate with nursing, therapy, the business office, and other disciplines regarding skilled services, coverage, and reimbursement. Participate in utilization review, triple-check meetings, and quality improvement activities. Provide staff education related to assessment and documentation requirements.

Qualifications:

Current, active, unencumbered RN license to practice in Alabama. Prior experience completing and coordinating MDS 3.0 assessments in a skilled nursing facility required. At least three years of RN experience, including one year of supervisory experience. Working knowledge of the RAI process, care planning, Medicare coverage requirements, and the Patient-Driven Payment Model (PDPM). Strong assessment, communication, organizational, and computer skills. Ability to work independently, manage deadlines, and collaborate with an interdisciplinary team. Current CPR certification. Resident Assessment Coordinator (RAC) training must be completed within six months of hire; RAC certification preferred.
