Schedule:

Monday Friday, 8:00 AM 5:00

PM Caseload:

2532 points per week (typically 35 patient visits per day) Required daily travel within the assigned area (mileage reimbursement provided)

Responsibilities:

Complete comprehensive OASIS assessments and create individualized care plans Deliver skilled nursing services, including wound care, medication administration, IV therapy, and management of chronic conditions Monitor patient status and communicate any changes or concerns to care teams and physicians Educate patients and their families on diagnoses, care routines, treatments, and medications Accurately document all care provided using the Home Care Home Base (HCHB) electronic system Coordinate care by collaborating with physical therapists, occupational therapists, social workers, and home health aides Participate in case conferences, staff meetings, and relevant training and education Adhere to all regulatory requirements and agency standards

Qualifications:

Active RN license in good standing At least 1 year of recent field-based Home Health experience Proficiency with Home Care Home Base (HCHB) and OASIS assessments is required BSN preferred Experience managing 2532 points per week and strong Medicare/home health standards knowledge is a plus Reliable transportation, valid drivers license, and auto insurance are required Excellent communication, clinical, and organizational skills Enjoy weekly pay, medical/dental/vision insurance options, 401(k) plans, 24/7 support, mileage reimbursement, and potential for contract extension.
