Nurse Navigator RN

Overview Responsibilities Develops and coordinates services provided to the oncology patient throughout the continuum of care in order to decrease fragmentation. Acts as a liaison between the physicians, the professional health care staff, and other healthcare team members to ensure quality and continuity of care delivery.

Qualifications Minimum Qualifications:
