Avanta Care is seeking a dedicated and detail-oriented Intake Registered Nurse (RN) to join our care team. The ideal candidate will be responsible for overseeing the admission process of new clients, ensuring all clinical and administrative documentation is accurate and complete, and coordinating with caregivers, clients, and healthcare providers to facilitate smooth service initiation.
Responsibilities:
Conduct comprehensive client assessments during the intake process. Review and verify all intake documentation, medical history, and care plans. Communicate with physicians, clients, and family members to ensure appropriate care coordination. Maintain accurate and confidential client records in compliance with agency and state regulations. Collaborate with scheduling and field staff to ensure timely assignment of services. Monitor client progress and update care plans as necessary. Provide clinical oversight and support to caregivers as needed. Ensure compliance with all agency policies and healthcare standards.
Qualifications:
Active and unrestricted Registered Nurse (RN) license in the state of Vermont. Minimum of 1 year of nursing experience, preferably in home health or a related field. Strong assessment, documentation, and organizational skills. Excellent communication and interpersonal abilities. Ability to work independently and manage multiple priorities effectively.
