Job Description

Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. We are seeking a skilled and compassionate Registered Nurse (RN) with solid home health and OASIS experience to join a dedicated care team for a 13-week full-time contract assignment. Candidates who excel in clinical care, precise documentation, and time management will thrive in this impactful role, making a difference in patients' lives every day.
Schedule:
13-week contract, with the potential for extension Full-time, Monday through Friday On-call may be required Structured daily home health visits
Key Responsibilities:
Deliver in-home nursing care and skilled interventions Complete patient assessments and OASIS documentation accurately and promptly Collaborate effectively with physicians and interdisciplinary team members to create and implement individualized care plans Provide wound care, manage medications, offer patient and caregiver education, and monitor chronic illnesses Maintain clear, timely communication with the clinical team regarding patient status and changes
Required Qualifications:
Current active RN license At least one year of recent experience as a home health RN Demonstrated proficiency with OASIS documentation HomeCare HomeBase software experience is strongly preferred Reliable transportation and a valid driver's license Strong organizational skills and ability to self-manage a full patient caseload
