An opportunity is available for a dedicated and compassionate Home Health Registered Nurse (RN) to deliver exceptional care to patients in the comfort of their own homes. This position is a full-time, 13-week contract assignment. PM Responsibilities:

Complete comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing interventions including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient status, promptly addressing and reporting any changes to the care team and physicians. Educate patients and their families regarding disease management, treatment regimens, medications, and preventive measures. Accurately document all care and communication within the Home Care Home Base (HCHB) electronic system. Collaborate effectively with other healthcare professionals (e.g., physical/occupational therapists, social workers, aides) to ensure coordinated and effective care. Attend case conferences, staff meetings, and participate in ongoing professional development. Ensure adherence to all regulatory policies and documentation standards.

Qualifications:

Active RN license in good standing within the state. At least 1 year of recent field-based Home Health RN experience. Proficiency with OASIS and Home Care Home Base (HCHB) is required. Strong clinical and assessment skills; excellent communication and organizational ability. Reliable vehicle, valid drivers license, and current auto insurance. Ability to independently manage a caseload of 2532 points per week. BSN degree and knowledge of Medicare/home health compliance standards are preferred.
