Deliver compassionate care where it matters most by providing skilled nursing services to patients in their homes. This 13-week, full-time contract offers the opportunity to work Monday through Friday, 8:00 AM 5:00 PM, with a manageable caseload of approximately 35 patient visits per day (30 points per week). As a field-based RN, you will play an essential role in assessing patient needs, coordinating care, and supporting recovery and wellness across your assigned territory. Mileage reimbursement is provided for daily local travel.

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans tailored to each patient. Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management. Monitor and evaluate patient progress, promptly communicating changes to the care team and physicians. Educate patients and families on disease processes, treatment plans, medications, and preventive health practices. Accurately document all assessments and interventions in Home Care Home Base (HCHB). Collaborate closely with interdisciplinary team membersphysical therapists, occupational therapists, medical social workers, and home health aides. Participate in case conferences, team meetings, and ongoing education initiatives. Ensure compliance with regulatory and agency standards in all aspects of patient care and documentation.

Required Qualifications:

Active RN license in good standing for the state of practice. Minimum of one year of recent Home Health experience (field experience highly preferred). Proficiency in Home Care Home Base (HCHB) and OASIS assessment tools. Reliable transportation, valid drivers license, and current auto insurance.
