This full-time, 13-week contract position specializes in providing skilled nursing support for patients in their own homes, ensuring continuity and quality of care throughout every step of their health journey.

Schedule & Setting:

Full-time, 13-week contract Monday Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (generally 35 patient visits each day) Field-based with daily travel to patient homes (mileage reimbursement included)

Desired Qualifications:

Active, unrestricted RN license in the state of practice At least one year of recent experience in Home Health (preferably with fieldwork) Proficient in Home Care Home Base (HCHB) Skilled in using OASIS for patient assessment and documentation Reliable transportation, current drivers license, and auto insurance required

Role Responsibilities:

Perform comprehensive OASIS assessments and develop individualized patient care plans Deliver skilled nursing interventions including wound care, medication management, IV therapy, and chronic disease support Monitor patient progress and promptly communicate status changes with physicians and the interdisciplinary care team Educate patients and families about conditions, medications, plans of care, and preventive strategies Ensure documentation is thorough and timely using HCHB Collaborate effectively with physical therapists, occupational therapists, social workers, and home health aides Engage in regular team meetings, case conferences, and ongoing professional development Uphold agency and regulatory standards for high-quality care
