Schedule:

Full-time, Monday through Friday On-call availability may be required

Qualifications:

Hold an active RN license Minimum of 1 year of recent experience in home health nursing Demonstrated OASIS documentation proficiency Familiarity with HomeCare HomeBase (highly preferred) Ability to manage daily patient visits and meet structured expectations Possess reliable transportation and a valid drivers license

Primary Responsibilities:

Deliver skilled in-home nursing care and clinical interventions Complete accurate, timely OASIS assessments and documentation Work closely with physicians and interdisciplinary teams on customized care plans Perform wound care, medication administration, patient education, and symptom monitoring Communicate patient condition changes and updates to the clinical team
