Job Summary:

Seeking an experienced Registered Nurse (RN) with Home Health/Hospice experience to provide compassionate patient care in the home setting. Responsibilities include hospice case management, patient assessments, admissions, interdisciplinary team participation, and travel between patient residences.

Key Responsibilities:

Provide skilled hospice nursing care in patients' homes Perform patient assessments and admissions Manage assigned hospice caseload Participate in IDG (Interdisciplinary Group) meetings Develop and coordinate patient care plans Travel daily between patient homes within the coverage area Ensure accurate and timely documentation in

HCHB Requirements:

Active RN license Associate Degree in Nursing (minimum) Home Health and/or Hospice experience HCHB (Homecare Homebase) experience required Strong communication and patient care skills Reliable transportation

Preferred:

Hospice admissions experience Hospice needs assessment experience Strong case management background
