Key Responsibilities:

Conduct home health risk assessments, including annual and post-discharge evaluations for Medicare Advantage plan members Assess patients prior and current medical history, as well as review existing medical conditions and medication usage Educate members about their medical conditions and prescription regimens Identify care gaps and offer recommendations for follow-up or further care Focus exclusively on assessmentsthere is no treatment planning or prescribing responsibilities

Qualifications:
