The MDS Coordinator will play a crucial role in ensuring accurate and timely completion of the Minimum Data Set assessments, supporting quality patient care and compliance with regulatory requirements. Responsibilities Complete and accurately document Minimum Data Set (MDS) assessments for residents in a timely manner. Collaborate closely with nursing staff, therapists, and other care team members to gather necessary information and ensure compliance with regulatory standards. Analyze resident care plans and outcomes to support quality improvement initiatives. Maintain up-to-date knowledge of federal and state regulations related to MDS documentation and reporting. Provide training and support to nursing staff on MDS-related processes and documentation. Communicate effectively with interdisciplinary team members to support resident care and regulatory compliance. Requirements Previous experience as an MDS Coordinator or in a similar role within a nursing home or long-term care facility. Strong knowledge of nursing home regulations, Medicare, and Medicaid requirements. Excellent attention to detail, organizational skills, and the ability to manage multiple priorities. Effective communication and interpersonal skills. Proficiency with electronic health records and MDS software. Relevant certification or training in MDS coordination preferred.
