Fort Smith, Van Buren, Alma; Crawford and Sebastian Counties, with portions of

Logan and Franklin Counties Contract Length:

Three-month contract, possibility of an extension

Schedule:

Monday-Friday, 8:00 AM - 4:30

PM On-Call Requirement:

One weekend every four weekends and one weeknight every other week

Pay Rate:

$53.00 - $55.90 per hour, mileage reimbursement

Weekly Hour Guarantee:

32 hours Responsibilities Complete skilled home health nursing visits throughout the assigned coverage area Perform comprehensive patient assessments, including OASIS assessments Monitor patient conditions and identify changes in health status Develop and implement individualized care plans Coordinate care with physicians and interdisciplinary teams Educate patients and caregivers on treatment plans and disease management Ensure accurate and timely documentation of all patient care activities Requirements Active Arkansas Registered Nurse (RN) license Home Health experience required Experience completing OASIS assessments Ability to manage patient visits independently Reliable transportation for local travel Strong assessment and documentation skills Preferred Skills Strong clinical judgment and patient assessment skills Experience managing a home-based patient population Excellent communication and care coordination abilities Ability to work independently in the field

Pay Details:
