Key Responsibilities:

Deliver skilled in-home nursing care, ensuring safety and comfort for every patient Complete thorough patient assessments and OASIS documentation accurately and promptly Collaborate with physicians and the care team to personalize and update care plans Perform wound care, medication management, disease monitoring, and provide patient/caregiver education Communicate patient status updates and changes to the clinical team efficiently

Qualifications:

Current, active RN license At least 1 year of recent RN experience in home health OASIS documentation experience required Proficiency with HomeCare HomeBase preferred Proven ability to maintain a structured schedule with daily patient visits Reliable transportation and current drivers license
