PM Key Responsibilities:

Conduct OASIS assessments and establish targeted care plans for home health patients. Deliver skilled nursing services, including wound care, IV therapy, medication administration, and managing chronic conditions. Monitor and evaluate patient progress, promptly address changes in health status, and coordinate communication with the wider care team and physicians. Offer patient and family education regarding disease management, medication regimens, and preventive practices. Ensure precise documentation using Home Care Home Base (HCHB). Work collaboratively with therapists, medical social workers, and aides to deliver comprehensive and coordinated care. Attend case conferences, participate in staff meetings, and pursue ongoing education as needed. Uphold regulatory requirements and standards for documentation and patient services. Travel daily within the assigned territory, with mileage reimbursement provided.

Qualifications:

Current RN license in good standing (state of practice required) At least one year of recent home health nursing experience (field-based preferred) Strong proficiency with Home Care Home Base (HCHB) and OASIS documentation Valid drivers license, reliable transportation, and active auto insurance
