Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members during home visits. Review patients medical and medication history to accurately evaluate prior and current health status. Educate patients about their health conditions and prescribed medications. Identify any care gaps, and make recommendations to providers for addressing them. No requirement to create treatment plans or prescribe medications.

Qualifications:

Nurse Practitioner license in Arizona (AZ) Board certification through AANP or ANCC Current CPR certification (BLS) Completion of an MSN degree from an accredited nursing school At least 6 months of current Nurse Practitioner experience preferred

Pay is structured at $80-$90 per completed evaluation, with an additional mileage stipend and compensation for administrative time.
