Job Description
As a Hospice Clinical Manager RN at Gentiva, you will lead an interdisciplinary team delivering compassionate, home-based end-of-life care. You'll oversee clinical operations, ensure high-quality, compliant hospice services, and support nurses through coaching, scheduling, and case review. Working closely with physicians, social workers, and chaplains, you will coordinate individualized care plans that honor patient dignity and comfort. This role is ideal for an experienced RN leader passionate about mentoring teams and improving patient and family experiences. Responsibilities Lead and support an interdisciplinary hospice clinical team delivering home-based end-of-life care. Oversee patient assessments, care planning, and ongoing evaluation for clinical quality and safety. Ensure compliance with hospice regulations, Gentiva policies, and documentation standards. Mentor and develop nursing staff through coaching, training, and performance feedback. Collaborate with physicians, social workers, and chaplains to coordinate holistic patient and family care. Monitor clinical outcomes and drive quality improvement initiatives. Participate in staffing, scheduling, and resource allocation to meet patient care needs. Serve as a clinical resource for complex cases, symptom management, and family education.
