DEFINITION/PURPOSE
: Responsible for providing direct community-based care to community members in collaboration with WMHC staff.

DUTIES AND RESPONSIBILITIES
- Conducts home visits to assess and provide patient care. Evaluates patients' condition and treatment plans. Makes referrals as needed. Consults with other health care professionals.
- Shares information about patient's condition and teaches health care assessment and decision process to CNAs and the CHR.
- Collaborates with WMHC staff, other Fort McDowell departments, state and federal health care agencies in providing comprehensive nursing care and programs.
- Conducts community assessments with WMHC staff to determine the needs of the community.
- A resource to the Certified Nursing Assistants, Community Health Representative, Drivers, RN Case Manager and other WMHC staff.
- Documents in the forms electronic health record and forms as needed.
- May provide supervision of community health staff as delegated.
- Actively participates in staff meetings, committees and special project work groups.
- Serves as community health manager in absence of the manager.
- Arranges and conducts vision/hearing screens and immunization reviews at tribal school.
- Work with other Tribal Government Departments as active participant on FMYN Wellness Court staffing and SANE duties.

SKILLS, ABILITIES AND KNOWLEDGE
Computer skills in word processing and data management. Maintains professional knowledge and proficiency in nursing through continuing education.
MINIMUM QUALIFICATIONS
1) Licensure in the State of Arizona as a Registered Nurse (RN); 2) Experience in dialysis, Home Health, public health nursing, medical/surgical nursing, community health or nursing home; 3) Fours years experience will substitute for a Bachelor of Science in Nursing; 4) Willingness to obtain SANE training, if without prior experience 5) Experience with an American Indian organization or tribe is desirable; 6) Current AZ drivers license with no restrictions and meet FMYN insurance standards.
PAYRATE:
$67,119.21 to $80,380.71 Per Annum (DOE)
