Job Description

Overview Join our compassionate healthcare team as a Hospice Registered Nurse Case Manager, where your expertise will make a meaningful difference in the lives of patients and their families during their most vulnerable moments. In this vital role, you will coordinate and deliver comprehensive hospice care, ensuring patients receive personalized, respectful, and high-quality support in their homes or care facilities. Your clinical skills, combined with your empathetic approach, will foster comfort and dignity for those facing life-limiting illnesses. This position offers an energizing opportunity to lead patient-centered care with a focus on comfort, symptom management, and emotional support. Duties Conduct thorough patient assessments to determine care needs, including vital signs, physiological status, and medical history. Develop and implement individualized care plans in collaboration with interdisciplinary teams, ensuring alignment with patient wishes and hospice guidelines. Manage symptom control and palliative interventions to maximize patient comfort, including medication management and non-pharmacological strategies. Coordinate discharge planning and post-acute care transitions to ensure seamless continuity of care across inpatient, outpatient, or home settings. Educate patients and families on disease progression, medication use, advanced directives, and available community resources. Perform regular patient visits—either in-home or facility-based—to monitor health status and adjust care plans accordingly. Maintain accurate medical records in compliance with HIPAA regulations while documenting all assessments, interventions, and communications thoroughly. Collaborate with physicians, social workers, chaplains, and other healthcare professionals to deliver holistic hospice services aligned with patient goals. Participate in utilization management activities by evaluating the appropriateness of services and supporting case reviews for quality assurance. Experience Proven experience in critical care environments such as ICU or emergency medicine settings; experience in Level I or Level II trauma centers is highly desirable. Strong background in hospital medicine or medical-surgical nursing with a focus on acute care and patient assessment skills. Prior post-acute or outpatient clinic experience working with diverse populations including geriatrics, pediatrics (PICU), behavioral health patients, or those requiring infectious disease care. Familiarity with managed care processes, discharge planning procedures, medical records documentation, and diagnostic evaluation techniques. Knowledge of physiology principles relevant to complex patient conditions; ability to interpret vital signs and clinical data accurately. Experience working within hospital or inpatient settings involving intensive intake procedures and utilization management strategies. Excellent communication skills for effective patient service delivery; ability to educate families about treatment options while maintaining HIPAA compliance. Demonstrated ability to handle high-pressure situations involving emergency room scenarios or trauma cases with professionalism and compassion.
