Job Description

About the
Role:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health and Hospice team. This position plays a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their homes. The ideal candidate is clinically skilled, empathetic, organized, and committed to supporting patients and families through both recovery and end-of-life care. The Registered Nurse will coordinate care plans, perform patient assessments, administer treatments, provide education to patients and caregivers, and collaborate closely with physicians and interdisciplinary teams to ensure continuity of care.
Key Responsibilities:
  • Perform comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing care in accordance with physician orders and organizational policies
  • Monitor patient condition and document clinical findings accurately and timely
  • Administer medications, treatments, and other nursing interventions as ordered
  • Educate patients and family members regarding diagnoses, medications, treatments, and care plans
  • Coordinate care with physicians, therapists, social workers, and other interdisciplinary team members
  • Participate in home health and hospice admissions, recertifications, and discharge planning
  • Ensure compliance with state, federal, and accreditation regulations
  • Respond appropriately to changes in patient condition and emergency situations
  • Maintain professionalism, compassion, and patient confidentiality at all times
Qualifications :
  • Current Registered Nurse (RN) license in the State of Arizona
  • Graduate of a National League for Nursing accredited school of nursing
  • Current CPR Certification required
  • Minimum of two (2) years nursing experience, at least one (1) year in public health, home health, or home care setting preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent verbal and written communication skills
  • Ability to work independently and manage time effectively
  • Compassionate approach to patient care and family support
  • Physically fit for prolonged walking, standing, and daily patient visits
  • Ability to lift and carry supplies, equipment, and other necessary materials as required
  • Valid driver's license, reliable transportation, and current auto insurance required
