Job Description

Sapphire Hospice is growing and hiring compassionate, self-directed PRN Registered Nurses to support our patients and families across the West Valley . This is a flexible, as-needed role covering triage, on-call, and case management based on patient needs. Ideal for an experienced hospice RN who stays calm under pressure, thinks on their feet, and wants meaningful work with schedule flexibility. What you'll do: Provide fee per-visit coverage during the week, or on-call, based on patient and staffing needs Triage patient and family calls, assess needs by phone, and prioritize response Provide RN case management , including a potential assigned caseload, paid on a PRN basis Conduct in-home visits for admissions, assessments, symptom management, and urgent changes in condition Support and update the plan of care and communicate promptly with the interdisciplinary team Complete accurate, timely clinical documentation Pronounce and manage end-of-life visits with compassion and professionalism What we're looking for: Active, unrestricted Arizona RN license At least 6 months of hospice experience required Strong triage, assessment, and case-management skills with confident clinical judgment Reliable transportation and valid AZ driver's license Availability for as-needed shifts across the week, including some evenings, weekends, or on-call A calm, solutions-oriented "let's find a way to say yes" approach to patient care
