Deliver high-quality care in a dynamic, patient-centered home health role serving Medicare Advantage plan members across Santa Cruz County, AZ, including the areas of Rio Rico, Tubac, and Sonoita. As a Nurse Practitioner, youll conduct home-based health risk assessments, contributing to preventive care and improved health outcomes for a diverse patient population.

Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate patients medical histories, current health conditions, and medication regimens Educate members about health conditions and medications, supporting patient understanding and engagement Identify care gaps and provide recommendations, promoting continuity and quality of care Each assessment visit averages 45 minutes; activities may include annual assessments or post-discharge follow-ups No responsibility for creating treatment plans or prescribing medications

Qualifications:

Current Nurse Practitioner license in Arizona Board certification through AANP or ANCC Valid CPR/BLS certification Minimum of six months experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution
