Key Responsibilities:

Perform annual risk assessments for a Medicare Advantage health plan Review and document detailed medical histories, including conditions and medication lists Offer patient education focused on disease management and medication usage Identify care gaps and recommend appropriate actionswithout treatment planning or medication prescribing responsibilities

Qualifications:
