We are currently recruiting for a Full-Time (Salary) RN Case Manager to serve patients and families in their homes in the Phoenix East Valley cities (Chandler, Sun Lakes, Gilbert and some area of Mesa.) Our agency branch is a place you will love to work - offering a true team culture with the hospice heart at the center of it. We are seeking quality visiting case managers who have felt "the calling" to end-of-life-care. Full-time employees at THEMA enjoy a comprehensive benefits package including medical/dental, LTD, life insurance, PTO, 401K. Pay is Salary, paid mileage. Responsibilities Provide compassionate, comprehensive hospice care to patients, visits to home or facility-homes, ensuring comfort and dignity in end-of-life situations. Conduct thorough patient assessments and develop individualized care plans in collaboration with interdisciplinary teams. Monitor patient progress and adjust care plans as necessary, adhering to HIPAA regulations. Educate patients and families about care options, symptom management, and resources available. Facilitate discharge planning and coordinate with healthcare providers to ensure continuity of care. Maintain accurate medical records and documentation in compliance with medical terminology standards.
