POSITION DUTIES

•Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc) in an inpatient setting.

MINIMUM REQUIRED QUALIFICATIONS

•CA RN license; BLS; 3 years' nursing experience in an acute hospital setting; Prior care coordination experience in a CommonSpirit/Dignity/CHI hospital

LENGTH OF ASSIGNMENT

•13 weeks

SHIFT / HOURS PER WEEK

•8hr days SYSTEMS•Cerner
