In this vital role, you will coordinate and oversee patient care plans within the home health setting, ensuring seamless delivery of high-quality nursing services. Your expertise will help patients achieve optimal health outcomes by providing personalized assessments, discharge planning, and ongoing support. Responsibilities Conduct comprehensive patient assessments, including vital signs, medical history review, and physiological evaluations to determine individual care needs. Develop and implement personalized care plans aligned with physician orders and patient goals, incorporating best practices in acute care, post-acute care, and outpatient settings. Manage patient discharge planning by coordinating with hospitals, clinics, and other healthcare providers to ensure smooth transitions of care. Monitor patient progress through regular visits and documentation of medical records, adjusting care strategies as needed to optimize recovery. Educate patients and their families on medication management, infection control, and health maintenance strategies tailored to geriatrics, pediatrics, behavioral health, or other specialized populations. Collaborate with multidisciplinary teams including physicians, therapists, social workers, and managed care professionals to facilitate comprehensive patient support. Ensure compliance with HIPAA regulations, medical terminology standards, and hospital policies while maintaining accurate documentation of patient interactions and case management activities. Requirements Valid LVN license in good standing with the state licensing board. Critical care experience with a strong background in ICU or emergency medicine environments. Experience working in Level I Trauma Centers or Level II trauma centers is highly desirable. Proven expertise in hospital medicine, medical-surgical units, or inpatient/outpatient settings such as clinics or post-acute care facilities. Knowledge of infectious disease care protocols and physiology fundamentals necessary for complex case management. Familiarity with diagnostic evaluation processes, utilization management, discharge planning, and medical records documentation. Strong patient assessment skills complemented by excellent communication abilities and a compassionate approach to patient service. Ability to work independently while collaborating effectively within multidisciplinary healthcare teams. Knowledge of HIPAA regulations and medical terminology relevant to diverse populations including pediatrics and geriatrics. Join us as a Home Health Case Manager
