We are looking for a individual with specific Home Health experience to join our office-based clinical team. Unlike a field role, this position focuses on the administrative and coordinative side of patient care, ensuring our field staff, patients, and families receive the support and clinical oversight they need to maintain high-quality life care.
Key Responsibilities Chart Reviews:
They review new admission and ongoing charts to identify missing diagnoses, undocumented procedures, or unclear information
Provider Queries:
Send queries to physicians to clarify diagnoses, conditions, or treatments to get the complete clinical picture.
Provider education:
Educate physicians and other providers ton how to improve their documentation practices, which help ensure correct coding and billing.
Auditing and quality improvement:
Conduct chart audits and contributes to quality improvement initiatives by ensuring data accuracy.
Quality Assurance & Review:
Conduct thorough audits of clinical documentation to ensure compliance with HIPAA regulations and hospice-specific Medicare guidelines.
Medication & Equipment Management:
Coordinate medication refills and Durable Medical Equipment (DME) orders, ensuring timely delivery for patient comfort.
Documentation:
Utilize EMR/EHR systems like maintain accurate and timely medical records.
Experience & Requirements Licensure:
Active Registered Nurse (RN) license in the state of California.
Technical Proficiency:
Strong knowledge of EMR systems like DSL . As well as being proficient in computer skills.
Communication:
Exceptional phone etiquette and the ability to explain complex medical information to families during emotionally sensitive times.
