This position is ideal for an LVN looking for a flexible schedule while providing one-on-one skilled nursing care to patients in their homes. Position Overview The LVN will provide skilled nursing services under the supervision and direction of a Registered Nurse (RN) and in accordance with each patient's established plan of care.

Per Visit Hours:

Approximately 5-20 hours per week, depending on patient/nurse availability

Service Area:

South Bay, Carson, Torrance, Gardena, Long Beach, and surrounding areas Patient visits are generally scheduled during daytime hours. The LVN will have flexibility to coordinate visit times with patients based on their availability and the patient's plan of care. Responsibilities Provide skilled nursing care to patients in their homes Perform patient assessments and monitor changes in condition Provide wound care and dressing changes as ordered Monitor vital signs and patient progress Communicate changes in patient condition to the supervising RN and agency Complete accurate and timely clinical documentation Follow physician orders and the established plan of care Maintain professional communication with patients, families, physicians, and agency staff Qualifications Current and valid California LVN license Current CPR/BLS certification Valid driver's license and reliable transportation Ability to travel to patients' homes within the assigned service area Strong communication and documentation skills Ability to work independently in a home health environment Dependable, organized, and comfortable managing a flexible patient schedule Preferred Qualifications Previous home health experience Wound care experience Experience working with Medicare and geriatric patients Bilingual English/Spanish is a plus Why Join Transitions Home Health?
