An opportunity is available for an experienced and compassionate Registered Nurse to provide skilled care to patients in their homes. This contract role focuses on delivering high-quality, patient-centered nursing, supporting both clinical outcomes and positive patient experiences. You'll independently manage your caseload and collaborate with a multidisciplinary healthcare team to achieve optimal results.

Monday Friday, 8:00 AM 5:00

PM Responsibilities:

Complete comprehensive OASIS assessments and develop individualized patient care plans. Deliver skilled nursing interventions, including wound care, medication administration, IV therapy, and chronic disease management. Evaluate patient progress, promptly report significant changes, and coordinate care with physicians and colleagues. Educate patients and families about disease management, treatment regimens, medications, and prevention strategies. Document all care in the Home Care Home Base (HCHB) electronic system in compliance with regulatory standards. Collaborate with therapists, social workers, and home health aides to coordinate services. Participate in case conferences, meetings, and ongoing educational opportunities. Travel daily within the assigned territory for in-home visits (mileage reimbursement provided).

Qualifications:

Current, active RN license in good standing. Minimum 1 year of recent field-based Home Health RN experience. Proficiency in using Home Care Home Base (HCHB) and OASIS assessment tools. Reliable transportation, valid drivers license, and auto insurance required. Strong clinical assessment, organizational, and communication skills. Ability to work independently with a flexible and responsive approach. BSN preferred; knowledge of Medicare and compliance standards a plus.
