A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.
Tri-City is seeking to hire two Registered Nurses (RNs): one to support the FACT Transitional Age Youth (TAY) program and one to support the ACT Adult Full Service Partnership program. The RNs will provide professional psychiatric nursing services in an outpatient community behavioral health setting, delivering safe, evidence-based, and client-centered care. As part of an interdisciplinary treatment team, each RN will support assessments, treatment planning, medication management, crisis response, and recovery-oriented services. Working within the Forensic Assertive Community Treatment (FACT) and Assertive Community Treatment (ACT) models, the RNs will help promote continuity of care, collaboration, and positive outcomes across the behavioral health continuum. These positions report to the Registered Nurse Supervisor.

They work well within an interdisciplinary team, remain calm during crises, and demonstrate strong skills in medication management, client education, clinical documentation, and trauma-informed care.
KEY DUTIES
Provide psychiatric nursing services in office, community, and field-based settings. Conduct nursing assessments and monitor clients' physical and behavioral health needs. Administer prescribed medications and injections while monitoring effectiveness, side effects, and medication adherence. Participate in treatment planning, case conferences, crisis response, and care coordination. Educate clients and families about medications, treatment plans, health maintenance, and available resources. Collaborate with psychiatrists, nurse practitioners, therapists, case managers, peer specialists, and community partners. Respond to changes in client conditions, behavioral health crises, and safety concerns. Maintain accurate and timely electronic health record documentation in accordance with Medi-Cal and regulatory requirements. Support trauma-informed, culturally responsive, and recovery-oriented services within the FACT program model.
EDUCATION, TRAINING, & EXPERIENCE
Education, Experience, and Licensure Requirements Completion of an accredited Registered Nursing program or Bachelor of Science in Nursing (BSN) program. At least one year of direct patient care experience in a clinical setting. Current and active California Registered Nurse (RN) license. Current Basic Life Support (BLS) certification from the American Heart Association or an equivalent recognized provider. Certification must be maintained throughout employment. Experience in psychiatric nursing, behavioral health, community health, or a related setting is strongly preferred. Experience working with individuals who have serious mental illness, co-occurring substance use disorders, or complex psychosocial needs is desirable. Ability to travel independently throughout the designated service area. When driving is required, possession of a valid California driver's license, an acceptable driving record, and current vehicle registration and insurance.
