Job Description
Patient Assessment & Care Planning Evaluate health conditions, monitor symptoms, and develop/implement personalized care plans in coordination with physicians and other healthcare providers Clinical Procedures Administer prescribed medications, perform wound care, manage IV therapy, change dressings, and monitor vital signs Health Education Teach patients and families proper home care techniques, medication management, and disease prevention strategies Coordination of Care Collaborate with doctors, physical/occupational therapists, and other professionals to ensure continuity of care . Documentation Maintain accurate, timely medical records and update providers on patient progress Support & Comfort Provide emotional support, ensure a safe and clean home environment, and assist with daily activities when needed i Specialized Care May deliver palliative care, post-surgical wound management, or chronic disease monitoring depending on the care plan
