Job Description Father's Heart Ranch- STRTP is seeking an experienced, caring and compassionate Registered Nurse or Licensed Vocational Nurse to provide care for 16 boys age 6 years to 12 years. The ideal individual for our program will work in conjunction with the psychiatrist, physician, clinician and other professionals to provide medication support services, health assessments, and first aid treatment for our clients. The nurse will also be responsible for ensuring that all documentation for psychiatric and medical care is accurate and complete. Nurse will also follow all policies and procedures in regards to health and welfare of the clients.
Qualifications Education:
Graduate from and accredited school for nursing. Current unencumbered California Registered Nurse of LVN licensure Bachelor's Degree (BSN) or Master's Degree (MSN) is preferred Current BLS, First Aid and CPR certification Knowledge and understanding ICD 10 coding Able to work in a fast-paced environment
Experience:
Knowledge and understanding of Court Ordered Consents for Medications (JV223) Ability properly store, track, and document information regarding medications including MAR's and MIR's Keen understanding of the 5 rights of medications and how to assist clients with medication self-administration. Knowledge and understanding of medications adverse reactions and interactions 1 or more years of experience providing health care services to children in a residential setting preferred. Knowledge of crisis management techniques; ability to react effectively in emergency situations. Ability to maintain confidentiality and ensure that HIPAA regulations are followed Responsibilities Review health records, complete a health assessment and coordinate medical services (within 30 days) for each client upon intake Plan and manage clients' medical care according to individual needs Ensure that there is documentation of all medications for new clients. Ensure that the clients authorized representatives receive copies of documentation regarding clients health care (injuries, illnesses and routine care). Document the height and weight of each client within 48 hours of intake and then on-going on a monthly basis. Verification of medications ordered and dispensed for each client including ordering of refills and communicating with pharmacy regarding any errors in medications dispensed Assist clients with self-administer of medications as directed by the physician and psychiatrist; train staff on proper procedures for assisting with self-administering and maintaining documentation of medications Immediately notify provider regarding any adverse side effects that a client is experiencing due to medication Accompany clients on routine and emergency medical visits including dental and eyecare Provide input regarding health care services to be included in each clients' treatment plan Evaluate clients when injury or illness occurs and ensure proper treatment when necessary Collaborate with providers regarding follow-up treatments Remove and properly dispose of any expired or discontinued medications Maintain accurate documentation of all clients' health care services Regular and punctual attendance Maintain a safe work environment Advocate for clients regarding medications support services and other medical & healthcare services Participate in Treatment Team Meetings and collaborate with multidisciplinary team regarding treatment plans and goal. Provide non-judgmental, non-discriminatory care Keeping all medical and medications records current and accurate. Delivery of person-centered, trauma informed care and services.
