Job Description

Overview We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will possess a strong commitment to patient care and demonstrate exceptional clinical skills. As a Registered Nurse, you will be responsible for providing high-quality nursing care in Home Health Care. You will work collaboratively with healthcare professionals to ensure the best outcomes for patients while adhering to established protocols and standards. Responsibilities Conduct comprehensive patient assessments and develop individualized care plans. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patient vital signs and report any changes in condition to the healthcare team. Provide education and support to patients and their families regarding health management. Collaborate with interdisciplinary teams to coordinate patient care effectively. Maintain accurate medical documentation in compliance with HIPAA regulations. Assist in procedures such as IV insertion, suturing, catheterization, and emergency interventions. Participate in quality improvement initiatives and adhere to NCQA standards. Provide emotional support and counseling for patients dealing with chronic illnesses or end-of-life care. Skills The following skills are essential or desirable for this position: Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks. Experience in critical care settings, including ICU or Level I/II trauma centers. Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and medical documentation practices. Competence in performing advanced nursing procedures such as dialysis, airway management, or ventilator care. Familiarity with specialties including ophthalmology, geriatrics, pediatrics, hospice & palliative medicine, and behavioral health. Strong assessment skills for acute pain management, triage, and emergency medicine situations. Ability to work with diverse populations, including individuals with developmental disabilities or memory care needs. Experience in home care settings or outpatient clinics is a plus. Excellent communication skills for effective patient education and team collaboration. Pay:
$140.00 - $160.00 per hour
