Sacramento North Key Responsibilities Complete patient assessments & update care plans Provide symptom & pain management Educate patients/families on care and medications Coordinate with physicians and interdisciplinary team Document timely, accurate clinical notes Manage assigned caseload; ensure safe care environment Participate in on call rotation Requirements Must have: Current RN license NLN accredited nursing program graduate CPR certification Valid driver's license & insured vehicle 1+ recent year of RN experience or BSN Ability to lift/position patients; frequent standing/walking

Preferred:

Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting. Hospice/home health/public health experience Why Join us?
