Job Description

Hospice of Humboldt is looking for a Case Manager Nurse to join our team. This position is 3/4-time approximately 32 hours a week and comes with full benefits. The Case Manager is responsible for identifying needs and coordinating care provided to the Hospice Unit of Care (patient, family, caregivers). The Case Manager RN provides direct nursing care in patient's place of residence, under the direction of the Clinical Manager and under physician's orders. Care is provided in collaboration with other Interdisciplinary Group members as described in the patients' Plan of Care.
Minimum Requirements are as follows:
Graduate of an accredited nursing program. Current California Registered Nurse license. Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program. Valid driver's license, proof of acceptable automobile insurance coverage, and reliable transportation. Reliable mobile phone with plan that provides good local coverage.
