Job Overview Join our dynamic healthcare team as a Registered Nurse Case Manager, where your expertise will drive exceptional patient outcomes through comprehensive care coordination and management. In this vital role, you will leverage your critical care, trauma center, and hospital medicine experience to assess patient needs, develop personalized care plans, and facilitate seamless transitions across inpatient, outpatient, and post-acute settings. Your proactive approach and in-depth knowledge of medical terminology, physiology, and healthcare regulations will ensure patients receive the highest quality of care while optimizing resource utilization. This position offers an exciting opportunity to make a meaningful difference in patients' lives by combining clinical excellence with compassionate case management. Responsibilities Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs. Coordinate multidisciplinary care plans for diverse patient populations, including those in
ICU, PICU
(Pediatric Intensive Care Unit), Geriatrics, Behavioral Health, and Infectious Disease care. Manage utilization review and discharge planning processes to ensure appropriate levels of care across inpatient, outpatient, and post-acute settings. Collaborate with hospital teams to facilitate diagnostic evaluations, medical records documentation, and treatment strategies aligned with HIPAA regulations. Oversee intake procedures for new patients in both hospital and clinic environments, ensuring accurate medical records and effective patient service delivery. Engage in patient education on health management strategies related to acute care conditions, primary care needs, and outpatient services. Monitor patient progress through continuous evaluation of vital signs and clinical data; adjust care plans accordingly to improve outcomes. Lead case management initiatives by coordinating with managed care organizations to optimize resource use while maintaining high standards of patient care. Support complex discharge planning efforts by liaising with post-acute providers and outpatient clinics to ensure smooth transitions and follow-up care. Maintain detailed documentation of all patient interactions, assessments, interventions, and outcomes within medical records systems. Qualifications Valid nursing license with current registration; Bachelor of Science in Nursing (BSN) preferred. Atleast 1 year of experience in Home Health Care setting. Experience in SOC patient intake and evaluation. Experience in emergency room settings with skills in rapid assessment and urgent intervention. Familiarity with infectious disease management and post-acute care coordination. Proficiency in medical terminology, diagnostic evaluation procedures, and medical records documentation. Demonstrated ability to perform comprehensive patient assessments including vital signs monitoring and intake evaluations. Knowledge of HIPAA compliance standards related to patient privacy and data security. Excellent communication skills for collaborating with multidisciplinary teams and engaging patients effectively. Ability to navigate utilization management processes efficiently while advocating for optimal patient outcomes. Prior experience in outpatient clinics or managed care environments is a plus; familiarity with discharge planning tools is beneficial.
