Asherah Home and Palliative Care Job Summary We are seeking a compassionate and skilled Registered Nurse (RN) to join our home health team. The RN will provide high-quality, patient-centered care in the comfort of patients' homes, focusing on improving health outcomes, promoting independence, and enhancing quality of life. Responsibilities Perform initial and ongoing patient assessments Develop and implement individualized care plans Administer medications and treatments as prescribed Monitor patient condition and report changes to the physician Provide patient and family education on disease management and care plans Coordinate care with interdisciplinary team members Complete accurate and timely clinical documentation in compliance with regulations Ensure adherence to agency policies, Medicare guidelines, and state regulations Qualifications Active Registered Nurse (RN) license in CA Minimum of 1 year nursing experience (home health or hospice preferred) Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Ability to work independently and manage time effectively Reliable transportation and valid driver's license Current CPR certification Preferred Qualifications Experience with OASIS documentation Knowledge of Medicare home health regulations EMR experience Why Join Us At Asherah home and Palliative, we are committed to delivering compassionate, high-quality care while supporting our clinicians with a collaborative and respectful work environment.
