Qualifications:
Active Registered Nurse (RN) license Recent and relevant experience in Case Management Strong assessment, organizational, and communication skills Ability to collaborate with interdisciplinary teams Familiarity with care transitions, discharge planning, and resource management
Responsibilities:
Coordinate and manage patient care through effective case management processes Develop individualized care plans and oversee patient progress Facilitate communication among patients, families, providers, and insurance representatives Ensure efficient utilization of healthcare resources Support patients through the transition between different care settings
