Job Summary We are seeking a highly skilled and detail-oriented MDS (Minimum Data Set) Coordinator to join our healthcare team. The ideal candidate will possess extensive experience in clinical documentation, medical coding, and utilization management within long-term care setting. As an MDS Coordinator, you will be responsible for ensuring accurate completion of MDS assessments, maintaining compliance with federal and state regulations, and supporting quality improvement initiatives to optimize resident care and reimbursement. Responsibilities Coordinate and facilitate the comprehensive assessment process by accurately completing the MDS and other required documentation in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines. Collaborate with interdisciplinary teams including nursing, therapy, case management, and medical staff to gather pertinent resident information for assessments. Ensure compliance with HIPAA regulations and state healthcare policies while managing sensitive medical records and electronic health records (EHR/EHR systems). Support discharge planning processes by providing accurate data for reimbursement and care planning purposes. Utilize Point Click Care to maintain precise documentation workflows. Stay current on health regulation policies including CMS guidelines, HIPAA compliance, and state-specific healthcare regulations to ensure ongoing adherence. Skills Strong understanding of managed care processes, Medicare regulations, ICD coding (ICD-9/ICD-10) and utilization review procedures. Excellent analytical skills with attention to detail for documentation review and medical records management. Effective communication skills to collaborate across multidisciplinary teams while maintaining compliance with HIPAA standards. Knowledge of physiology and anatomy relevant to accurate clinical assessment documentation.
