Oasis Home Health Care La Puente, CA Job Details Part-time | Full-time $2,000 - $3,000 a month 11 hours ago Qualifications Hospice experience HIPAA Health information management Hospice & palliative medicine Medical terminology Full Job Description Overview We are seeking a dedicated and experienced DON Registered Nurse for Home Health to lead our nursing team in providing exceptional patient care within the home setting. This vital role involves overseeing clinical operations, ensuring compliance with healthcare regulations, and fostering a compassionate environment for patients and their families. As a key leader, you will coordinate care plans, supervise nursing staff, and uphold the highest standards of nursing practice to promote health, recovery, and well-being in the community. Duties Lead and manage the home health nursing team, providing guidance, supervision, and mentorship to ensure delivery of quality patient care. Develop, review, and update individualized care plans in collaboration with physicians, patients, and families to meet specific health needs. Oversee documentation review and ensure accurate medical records management using Electronic Health Record (EHR) systems while maintaining compliance with HIPAA regulations. Coordinate shift management and staffing schedules to optimize patient coverage and staff workload. Monitor patient progress through assessments of physiology and anatomy knowledge, adjusting care strategies accordingly. Ensure adherence to Medicare guidelines, regulatory standards, and nursing best practices across all services including hospice care, assisted living, nursing homes, and home health. Supervise the management of MDS (Minimum Data Set) assessments to support quality reporting and reimbursement processes. Review documentation for accuracy in medical terminology, care plans, and medical records to ensure compliance with healthcare policies. Foster leadership within the nursing team by providing supervision experience opportunities and supporting nurse management initiatives. Skills Extensive experience in assisted living facilities, nursing homes, hospice care, or home health environments demonstrating strong clinical expertise. Proven supervisory experience with a focus on nursing administration and shift management. Proficiency with EMR (Electronic Medical Records) systems and EHR (Electronic Health Record) systems for documentation review and record-keeping. Deep understanding of medical terminology, physiology, anatomy, and healthcare regulations including HIPAA compliance. Knowledge of Medicare requirements related to home health services and reimbursement processes. Strong leadership skills with the ability to manage teams effectively while fostering a collaborative work environment. Excellent communication skills for coordinating with multidisciplinary teams, patients, families, and healthcare providers.
