The ideal candidate will possess extensive clinical expertise, including critical care, emergency medicine, and hospital experience, with a focus on delivering compassionate, patient-centered care. As a Home Care Nurse, you will be responsible for assessing patient needs, developing care plans, and ensuring the highest standards of safety and quality in a home setting. This role offers an opportunity to make a meaningful difference in patients' lives by providing expert nursing care in a comfortable environment. Duties Collect vital signs, medical history review, and physiological evaluations to determine individual care needs. Manage daily life aspects (ADLs), including catheter care, bowel program, reposition, transfers via hoyer lyft, Administer medications, treatments, and diagnostic evaluations while adhering to HIPAA regulations and medical terminology standards. Monitor patient progress and daily tasks through the EMR system called Axxess. Educate patients and their families on health management, medication adherence, infection control (including infectious disease care), and safety protocols. Collaborate with multidisciplinary teams including physicians, therapists, and social workers to optimize patient outcomes across inpatient, outpatient, and clinic environments. Qualifications Demonstrated proficiency in patient assessment techniques, vital signs monitoring, and physiological evaluation. Familiarity with medical records systems and discharge planning procedures; ability to navigate diagnostic evaluations effectively. Excellent communication skills with the ability to educate patients on health management strategies effectively. Valid nursing license with current certification; additional certifications in trauma or emergency nursing are advantageous. Ability to work independently within managed care environments while maintaining high standards of patient service and safety.
