Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities. To provide intermittent visits to the Hospice and Palliative patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the end of life. Essential Functions and Responsibilities of the Job 1. Assesses present and potential needs of patient and family 2. Review Plan of Care with patient, family, physician, IDT and review/evaluate/modify routinely 3. Establish realistic goals of care to ensure continuity and quality of care 4. Complete all documentation in a concise, timely manner per agency standards 5. Manage symptoms and teach patient/family care management 6. Be at work and be on time 7. Follow company policies, procedures and directives 8. Interact in a positive and constructive manner 9. Prioritize and multitask •Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Qualifications:
Minimum Qualifications Work Experience:
Minimum 3 year's relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings EMR experience
Education/Licensure/Certification:
Current RN license issued by the California Board of Registered Nursing BSN preferred BLS issued by the American Heart Association Hospice and Palliative Care Certification preferred
