A Registered Nurse provides medical care and treatment, educates patients about their conditions, and provides emotional support to patients and families. Works in hospitals, schools, clinics or community health centers. Works under the supervision of a physician or other specialist, and serves as the primary point of care for patients in a hospital setting. May specialize in emergency room work, or treatment for a particular condition, or specialize in working with infants, the elderly or other patient groups. May work as an advanced practice specialists and prescribe medications in addition to offering specialty care.

Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. To provide intermittent visits to the Hospice and Palliative patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the end of life. Essential Functions and Responsibilities of the Job 1. Assesses present and potential needs of patient and family 2. Review Plan of Care with patient, family, physician, IDT and review/evaluate/modify routinely 3. Establish realistic goals of care to ensure continuity and quality of care 4. Complete all documentation in a concise, timely manner per agency standards 5. Manage symptoms and teach patient/family care management 6. Be at work and be on time 7. Follow company policies, procedures and directives 8. Interact in a positive and constructive manner 9. Prioritize and multitask •Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Qualifications:
Minimum Qualifications Work Experience:
Minimum 3 year's relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings EMR experience
Education/Licensure/Certification:
Current RN license issued by the California Board of Registered Nursing BSN preferred BLS issued by the American Heart Association Hospice and Palliative Care Certification preferred
