Job Description
Family Aid Hospice & Palliative Care Inc. is looking for
PER DIEM/PRN LICENSED VOCATIONAL NURSES
in the Los Angeles, San Fernando Valley, Antelope Valley, San Gabriel Valley, Glendora, Orange County and Ventura County areas, experienced in doing ADMISSION/s and Regular visit/s. Foreign Language speaking is a plus! Our hospice services are designed to provide compassionate care and support to individuals who are facing a life-limiting illness or condition. Our team of healthcare professionals, including doctors, nurses, and support staff, work together to address the physical, emotional, and spiritual needs of patients and their families. Our goal is to help patients live their remaining days with dignity, comfort, and quality of life. Hospice LVN is expected to report to clinical supervisor and other members of IDG including but not limited to: Administrator, DPCS, Case manager, Registered nurse, and Social worker. The Hospice LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, and participates in the care plan development and implementation. This position requires commute to various locations to provide individualized care to our patients.Job functions and responsibilities:
Provides services requiring nursing skills in accordance with the plan of care and as permitted by scope of practice. Reports changes in the patient's conditions and needs to the RN Case Manager or Director of Patient Care Services. Administers prescribed treatments and medications as permitted by scope of practice. Educates and provides individualized care instructions specific to patient and his/her family. Demonstrates flexibility in patient scheduling and readily accepts new assignments. Observes and abides safety hazards in the patient's and organization's environments and takes appropriate steps to reduce or eliminate them. Adheres to the highest standards of professional and personal conduct in the performance of his/her duties. Maintains continuing education through academic studies, seminars, and workshops (in-services, staff meetings and case conferences). Completes all documentation in an accurate, timely, and legible manner. Complies with hospice policies and procedures, laws and regulations and standards of practice.Job Types:
