Full Job Description Job Summary We are seeking a dedicated and compassionate Hospice Visit Registered Nurse (RN) to provide exceptional end-of-life care in patients' homes and community settings. The ideal candidate will possess strong clinical skills, a deep understanding of palliative and hospice care principles, and a commitment to enhancing patient comfort and quality of life. As a Hospice Visit RN, you will coordinate patient assessments, deliver personalized nursing interventions, and collaborate with interdisciplinary teams to ensure comprehensive, respectful, and compassionate care. Responsibilities Conduct thorough patient assessments during home visits, including vital signs, symptom management, and psychosocial evaluations. Develop individualized care plans aligned with hospice standards and patient preferences. Administer medications, including pain management therapies such as opioid titration and Botulinum toxin treatments when appropriate. Provide education to patients and families on disease progression, symptom control, medication administration, and advanced directives. Monitor and document patient progress accurately within electronic health record (EHR) systems such as Epic or Cerner. Coordinate with interdisciplinary teams—including physicians, social workers, and therapists—to ensure holistic care delivery. Manage acute symptoms such as pain or respiratory distress using aseptic techniques and appropriate interventions. Assist with complex procedures like IV infusions, catheter care, wound management, suturing, and ostomy care as needed. Ensure compliance with HIPAA regulations and maintain confidentiality of patient information at all times. Participate in ongoing training related to critical care, trauma medicine, infectious disease care, dementia care, and other specialized areas relevant to hospice patients. Qualifications Valid RN license with current registration; experience in hospice & palliative medicine preferred. Demonstrated proficiency with EMR/EHR systems such as Epic or Cerner; experience with medical documentation is essential. Strong clinical background in acute care settings including ICU, emergency medicine, or Level I trauma centers; critical care experience highly desirable. Knowledge of complex medication management including IV infusion therapy, pain management protocols, and Botulinum toxin treatment is advantageous. Familiarity with medical coding systems such as
ICD-10/ICD-9
for accurate documentation. Experience working with individuals with disabilities or developmental disabilities; familiarity with long-term care environments such as nursing homes or assisted living facilities is beneficial. Excellent communication skills for triage calls and patient education; ability to work independently in home settings while maintaining safety protocols. Additional skills in laboratory specimen collection/processing, infection control practices, and basic life support (BLS) certification are preferred.
