Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

In-office processing referrals for home health and hospice.

MINIMUM REQUIRED QUALIFICATIONS

Home health or hospice experience within the last three years, CA license

PREFERRED QUALIFICATIONS

Kaiser experience
