Company Will Be:
The RN travels to a patient's home to administer their services and helps patients maintain their independence.
Their duties include:
Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, As well as Updating Doctors & The Agency about their patient's health. Documentation and communication with the Agency is done through Synergy Software.
AVAILABILITY
Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

RESPONSIBILITIES
Providing nurse visits to home health patients. (home visits) Performing an initial, comprehensive assessment. Documenting everything & Uploading to Agency Portal Obtaining and documenting physician orders. In consultation with the assigned Clinical Team Manager, you will need to initiate and regularly re-evaluate and revise the plan of care Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient's condition and needs. Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completing and submitting all related documentation Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA). Performing Home Health Aide supervisory visits per regulatory requirements. Demonstrate knowledge of regulatory requirements.
