Pay Range:

$66/hour

Required Qualifications:

Must have 2-years acute Hospital Case Manager experience performing Care Coordination, Discharge Planning in a very fast paced environment. Must have Teaching Hospital Experience. Must have Community Hospital Experience. Must have LTAC Experience. Must have Trauma Level I & II Experience. Must have Travel Experience. Must have active State License. Must have BLS from AHA. Must have active Driver's License.

Must have Charting system:

Epic experience required. Must be flexible Weekend Rotation . Must have complete dose of MMR and Varicella/Titer Vaccine.
