We are privately owned, CHAP Accredited Home Health Agency based in Campbell, California. We provide care across the greater Bay Area and are currently hiring per diem, part time RN's for Napa County. This position will rapidly grow into Full Time. Our founders include 150+ years of highly experienced nurses, healthcare administrators and other professionals in the post-acute industry dedicated to the highest quality clinical outcomes and patient satisfaction.

Compensation:

$70-77 DOE on Oasis home healthcare experience We are seeking dedicated and compassionate Home Health RN(s) for our Napa territory to join us in our dynamic healthcare team at our Concord location. If you are a RN with a passion for providing quality care in the comfort of patients' homes, we invite you to apply for this position.

Responsibilities:

• Conduct comprehensive assessments of patients' needs and develop individualized care plans
• Collaborate with interdisciplinary teams to coordinate and facilitate patient care
• Monitor and evaluate patients' progress towards achieving their goals
• Provide education and support to patients and their families regarding treatment options and resources
• Advocate for patients' rights and ensure they receive appropriate services
• Maintain accurate and up-to-date documentation of patient interactions and interventions

Qualifications:

• Bachelor's degree in nursing
• Current licensure or certification in a relevant discipline (e.g., RN, LCSW)
• Experience working in a healthcare setting, preferably in emergency room, critical care, behavioral health, or ICU
• Strong assessment, communication, and problem-solving skills
• Ability to work independently and as part of a team
• Knowledge of community resources and services for patient referrals
• Proficiency in electronic medical records systems
• Home Health experience (1+ year)
