Napa County About the Role We are seeking a Registered Nurse (RN) to provide continuous, one-to-one nursing care in the patient's home, typically through 8-hour shifts . This role is ideal for an RN who operates with great attention to detail and a big heart, delivering high-quality, compassionate care while remaining attentive to changes in the patient's condition. In this role, you will focus on direct patient care, comprehensive assessment, and clinical escalation within the patient's home environment. You serve as the primary link between the patient and the clinical team, ensuring continuity of care and clinical excellence in accordance with: California Title 22 HHA regulations and CDPH standards. Accreditation Commission for Health Care (ACHC) standards. VA/TRICARE, Medi-Cal/CCS, and private insurance requirements. Key Responsibilities 1. Clinical Excellence & Patient Care Perform initial and ongoing physical assessments and histories to determine home care needs. Deliver direct, skilled nursing care including medication administration and treatments as prescribed by the physician. Initiate the plan of care and make necessary revisions as patient status and needs change. Develop care plans that establish goals based on nursing diagnosis and incorporate therapeutic, preventive, and rehabilitative nursing actions. Provide health care instructions and counseling to patients and families to support home-based stability Function as a case manager/supervisor when assigned by the Clinical Supervisor. 2. Communication & Coordination Prepare timely clinical notes and update the primary physician regarding condition changes at least every 60 days. Communicate with community health-related persons to coordinate a holistic care plan. Identify discharge planning needs during the care plan development and implementation. Supervise and evaluate home health aide care provided every two (2) weeks when assigned. 3. Compliance & Administrative Standards Ensure necessary equipment and supplies are available for patient care. Participate in on-call duties as defined by agency policy. Maintain strict adherence to CMS, ACHC, and Title 22 standards. Position Qualifications Active California RN license and current CPR certification. Graduate of an accredited school of nursing; BSN is preferred (not required). Minimum of one (1) year nursing experience in acute care or equivalent experience is preferred. Case management/supervisor only when assigned Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; competency matched to patient acuity. Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills. Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements Why Join Us?
